DM Care糖訊通」提供糖尠護理的相關資訊，包括「認識糖尠」、「看顧技巧」、「日常生活」和「同行者」，用戶也可輸入個人血糖資料和相關資訊作記錄分析。另外，用戶可在程式設定量度血糖及覆診提示，程式會根據設定發出提示通告。
(備註: 此程式現正在新界東聯網醫院(NTEC)包括威爾斯親王醫院(PWH)、大埔那打素醫院(AHNH)和北區醫院(NDH)試行推廣及只提供中文介面和內容。)
