En diciembre de 1531 la Virgen se aparece varias veces al indio Juan Diego en el Monte Tepeyac, de sólo cuarenta metros de altura, hoy en la actual capital de México, Distrito Federal. Le pide que quiere allí un templo en el llano. Que se lo diga al Obispo. Y le añade: «Como Madre, allí mostraré mi clemencia amorosa para todos los que soliciten mi amparo. Y oiré sus lágrimas y sus ruegos para darles consuelo y alivio. Porque soy vuestra Madre compasiva».

Fray Juan de Zumárraga, primer Obispo de México, de la Orden Franciscana, recibe amablemente al indio. Pero le dice que necesita una prueba para estar seguro de que lo que le dice es verdad. Como señal, la Virgen le dice al indio que suba a la colina, coja unas rosas y se las lleve al Obispo. Era diciembre. Juan Diego no lo dudó. Allí estaban las rosas. Las recogió en la tilma y se las llevó al Obispo como señal de que Ella quería allí un templo. La tilma, era el nombre en «náhuatl», la lengua que hablaba Juan Diego, del poncho o capa que utilizaban los indios pobres mexicanos, anudada al hombro.

Cuando Juan Diego está delante del Obispo y suelta la tilma donde llevaba las rosas, éstas cayeron al suelo. Como no era tiempo de rosas, el Obispo comprendió que la señal era verdadera. En la tilma apareció estampada la imagen de la Virgen. Esto ocurrió el 12 de diciembre de 1531.El Obispo emocionado, tomó en sus manos la tilma de Juan Diego y la colocó en su oratorio.
