Injury Report

One of my teacher friends is also the schools Athletic Trainer, and she asked if there was an app that makes the injury reporting process easy. I teach astronomy, so I had no idea. I searched for one, and found three. One was $100, another was $1.99, and the third was free, but didn't work and all of them gave me concerns about security, and privacy issues. I obsessed over it for a day, and figured that a good way to do it is through Google forms. I just had to connect it to an app. That's what I did. I made a small app that you will enter your Form address. This keeps all of the private data in your hands. I then created tags in a .doc that can convert the spreadsheet into a nice report. To make it work (slightly technical, but easily doable), you must make a copy of the Form I made. Just follow the instructions on my site. If you accidentally use my form without copying it, your data will go to my account. If you change the Form without copying it, you will change my Form. Any data you find in my form is fictitious, and is created only as a means to test it.
Once you have the copy made, you then add it to the app. The app will remember it, and will open your Google Form when you open the app. It will remember the page you entered. If you submit an empty box, it will clear that memory and you'll have to resubmit your form. Once you complete the form, the data is accessible from your Drive account. This app simply eliminates a step to open the Google Form. The work was not done creating the app, but in developing the Google Form
