Symptom Tracker by Northwestern University

The Symptom Tracker: Northwestern University is a short health questionnaire which must be taken daily and/or prior to all members of the community and general public -- student, staff, faculty, visitor, contractor, or other - attending on-campus programming or activities including classes. It will reveal any potential symptoms which would indicate that someone cannot visit or come onto campus. If the user is symptom-free, the green screen will indicate they are welcome to come onto campus and go about their activities as they wish. For the general public, a purple badge indicates that they are welcome to engage in their business or activities on-campus. If the user is experiencing symptoms, the red screen will let them know not to come onto campus and give them information on next steps.
