Las calientísimas arenas del desierto de Egipto afectaron la vista de Francisco hasta el punto de estar casi completamente ciego. Los dos últimos años de la vida de Francisco fueron de grandes sufrimientos que parecía que la copa se había llenado y rebalsado. Fuertes dolores debido al deterioro de muchos de sus órganos (estómago, hígado y el bazo), consecuencias de la malaria contraida en Egipto. En los más terribles dolores, Francisco ofrecía a Dios todo como penitencia, pues se consideraba gran pecador y para la salvación de las almas. Era durante su enfermedad y dolor donde sentía la mayor necesidad de cantar.

Su salud iba empeorando, los estigmas le hacían sufrir y le debilitaban, y casi había perdido la vista. En el verano de 1225 estuvo tan enfermo, que el cardenal Ugolino y el hermano Elías le obligaron a ponerse en manos del médico del Papa en Rieti. El Santo obedeció con sencillez. De camino a Rieti fue a visitar a Santa Clara en el convento de San Damián. Ahí, en medio de los más agudos sufrimientos físicos, escribió el "Cántico del hermano Sol" y lo adaptó a una tonada popular para que sus hermanos pudiesen cantarlo.

Después se trasladó a Monte Rainerio, donde se sometió al tratamiento brutal que el médico le había prescrito, pero la mejoría que ello le produjo fue sólo momentánea. Sus hermanos le llevaron entonces a Siena a consultar a otros médicos, pero para entonces el Santo estaba moribundo. En el testamento que dictó para sus frailes, les recomendaba la caridad fraterna, los exhortaba a amar y observar la santa pobreza, y a amar y honrar a la Iglesia. Poco antes de su muerte, dictó un nuevo testamento para recomendar a sus hermanos que observasen fielmente la regla y trabajasen manualmente, no por el deseo de lucro, sino para evitar la ociosidad y dar buen ejemplo. "Si no nos pagan nuestro trabajo, acudamos a la mesa del Señor, pidiendo limosna de puerta en puerta".

Cuando Francisco volvió a Asís, el Obispo le hospedó en su propia casa. Francisco rogó a los médicos que le dijesen la verdad, y éstos confesaron que sólo le quedaban unas cuantas semanas de vida. "¡Bienvenida, hermana Muerte!", exclamó el Santo y acto seguido, pidió que le trasportasen a la Porciúncula. Por el camino, cuando la comitiva se hallaba en la cumbre de una colina, desde la que se dominaba el panorama de Asís, pidió a los que portaban la camilla que se detuviesen un momento y entonces volvió sus ojos ciegos en dirección a la ciudad e imploró las bendiciones de Dios para ella y sus habitantes. The calientísimas desert sands of Egypt affected the view of Francis to the point of being almost completely blind. The last two years of the life of Francis were of great suffering that it seemed that the cup was filled and overflowed. Severe pain due to the deterioration of many of their organs (stomach, liver and spleen), consequences of malaria contracted in Egypt. In the most terrible pains, Francisco offered to God all penance, because it was considered a great sinner and for the salvation of souls. His health was getting worse, they made him suffer stigma and weakened, and had almost lost his sight. In the summer of 1225 he was so sick that Cardinal Ugolino and Brother Elias was forced to put in the hands of the doctor of the Pope in Rieti. The Holy obeyed simply. Way to Rieti went to visit Santa Clara in the convent of San Damiano. In the will he dictated to his brothers, he recommended fraternal charity, exhorting them to love and observe holy poverty, and to love and honor the Church. Shortly before his death, he dictated a new will to recommend to their brothers who faithfully trabajasen observasen the rule manually, not by the desire for profit, but to avoid idleness and a good example. "If you do not pay us our work, we turn to the Lord's table, begging from door to door".

When Francis returned to Assisi, Bishop hosted him in his own home. Francisco begged doctors to be told the truth, and they confessed that they had only a few weeks. "Welcome, Sister Death!" Exclaimed the Holy and then, he asked to trasportasen to the Portiuncula. By the way, when the party was at the top of a hill, from which the panorama of Assisi is dominated, he asked those who carried the stretcher a moment detuviesen and then turned his blind eyes toward the city and he implored God's blessings for her and her inhabitants.
